How quickly can someone know that something has happened? If the device works perfectly but Grandpa refuses to wear it, does the technology work? What does it actually take for fall-detection technology to work in real life?

Sensors|2025 — Fall Detection in Elderly People: A Systematic Review of Ambient Assisted Living and Smart Home-Related Technology Performance
原文链接:pmc.ncbi.nlm.nih.gov/articles/PMC12609574/

CDC|2026 — Older Adult Falls Data

Detection can shorten the time before help arrives — but only if the fall is detected, the device is worn, and the alert reaches someone.

Sensors 2025、Frontiers in Public Health 2026、CDC 2026、Journal of Geriatric Physical Therapy 2014、Measurement 2026、BMC Geriatrics
Can a Watch Catch a Fall?

Falls are one of the most serious threats to independence in later life. For an older person, a fall may lead to a broken hip, head injury, hospitalization or fear of falling again. In some cases, the danger is made worse when the person cannot get up and remains on the floor for a long period before anyone notices.

This has created growing interest in automatic fall-detection technology. A wearable device such as a smartwatch can use motion sensors, including accelerometers and gyroscopes, to measure changes in movement, speed and body orientation. If a pattern resembles a fall, the device may trigger an alarm.

Some commercial smartwatches can go further. Apple, for example, states that supported Apple Watch models can detect a hard fall, alert the wearer and, if the person remains immobile for about a minute, attempt to contact emergency services. The company also warns that the device cannot detect every fall and may mistake some high-impact activities for falls.

That warning reflects a basic challenge in medical detection systems. The technology must distinguish a dangerous event from normal daily activity. If the system is too sensitive, sitting down quickly, jumping or exercising may produce false positives. If it is too conservative, a genuine fall may become a false negative. Neither problem is trivial. Too many false alarms can make users stop trusting the system. Too many missed falls can create a dangerous sense of security.

Research results can also look more impressive in the laboratory than in real life. For years, many fall-detection systems were tested by asking young, healthy volunteers to perform controlled falls onto mats. These experiments are useful because they allow researchers to collect large amounts of data safely. But a planned laboratory fall is not necessarily the same as an unexpected fall experienced by an 82-year-old person at home.

A systematic review found that only a small percentage of wearable fall-detection projects had actually monitored older adults in real-world conditions. Many systems had instead been developed using simulated falls or younger participants. Newer reviews show the same general challenge. Algorithms may report excellent sensitivity and specificity in controlled environments, yet performance can fall when researchers test them across different datasets or move from simulated to real-life events. This is known as a simulation-to-reality gap.

There is another distinction that often disappears in advertising: fall detection is not fall prediction. Detection asks: "Has a fall just happened?" Prediction asks: "Who is likely to fall in the future?" Wearable devices can measure walking patterns, balance and movement over time to estimate future risk. But a 2026 meta-analysis found only moderate sensitivity for such prediction, meaning many future fallers could still be missed. Technology therefore works better as one part of fall-risk assessment than as a perfect forecast.

And accuracy is not the only challenge. A device cannot protect someone if it is sitting on the charger. Studies of older adults using wearable technology repeatedly identify comfort, ease of use, battery charging, cost, appearance and perceived usefulness as important factors in long-term adoption. Some older adults dislike devices that make them look frail. Others worry about GPS tracking or cameras inside their homes. Some prefer human support to continuous technological monitoring.

A 2024 study involving older adults and family members found that acceptance of fall-detection technology depended not only on safety, but also on comfort, affordability, habits and whether the technology supported rather than replaced human care. This leads to a broader lesson. A fall detector is not simply an algorithm. It is part of a human system. The person must agree to use it. The device must be charged and worn correctly. The algorithm must recognize the event. The network must transmit the alert. Someone must respond. Technology can make help faster. But safety depends on every link in the chain.

Fall detection — 回答:Did a fall happen? 技术目的是尽快发现,缩短 time on the floor,及时求助。
Fall prevention — 回答:How can we reduce the chance of falling in the first place?

Question 1
Which statement correctly distinguishes detection from prevention?
A. Detection identifies an event that has occurred, while prevention aims to reduce the chance that it occurs.
B. Detection and prevention are exactly the same process.
C. Fall detection guarantees that nobody is injured.
D. Fall prevention begins only after a person has fallen.
Answer: A

Question 2
Which measure is most directly concerned with how many real falls are successfully detected?
A. Sensitivity
B. Specificity
C. Battery life
D. Prevalence
Answer: A
55% sensitivity

Question 3
What does a pooled sensitivity of 0.55 suggest?
A. The devices correctly predict every future fall.
B. Around 55% of those who later fall are identified as positive by the models in the pooled analysis.
C. 55% of older adults will fall.
D. The devices have 55% battery life.
Answer: B

It looks like you've taken a hard fall.

Question 4
Why can too many false alarms reduce real-world safety?
A. Users may lose trust in the device, ignore alerts or stop using it.
B. False alarms directly cause every future fall.
C. They improve sensitivity automatically.
D. Emergency systems prefer false alarms.
Answer: A

这就出现一个很重要的概念:false reassurance(错误安全感)。 How reliable is the feature in the situation that matters?

Question 5
Which statement best describes a simulation-to-reality gap?
A. A system performs well under controlled tests but less reliably in real-world situations.
B. Simulated falls are always identical to real falls.
C. Laboratory experiments have no scientific value.
D. Older adults cannot participate in research.
Answer: A

所以 high laboratory performance 不能自动推出 high real-world performance in older adults。

LAB:Young volunteer ↓ "Fall now" ↓ Soft mat ↓ Repeated controlled movement ↓ 95%+ performance
REAL LIFE:Older adult ↓ Unexpected loss of balance ↓ Different surfaces ↓ Different fall patterns ↓ Device not always correctly positioned
This looks like a fall.
This person may be at higher risk of falling next month.

Never transfer a performance number to a different task.

Question 6
Why would it be incorrect to say "smartwatches detect only 55% of falls" based on the 2026 meta-analysis?
A. The meta-analysis evaluated prediction of future fall risk, not immediate detection of every fall event.
B. Sensitivity cannot be measured.
C. Smartwatches always detect 100% of falls.
D. The study included only children.
Answer: A

An unworn wearable has zero sensitivity.

Question 7
Which conclusion is most reasonable?
A. Older adults reject wearables mainly because they cannot understand technology.
B. Adoption depends on usefulness, usability, motivation and fit with everyday life.
C. More features always increase adoption.
D. Users should adapt to whatever engineers design.
Answer: B

Question 8
Which statement best reflects a balanced approach to safety and privacy?
A. Constant full surveillance is the only way to keep older adults safe.
B. Design can seek a balance between safety, autonomy and privacy rather than choosing one.
C. Privacy is unimportant when a person is old.
D. Older adults should accept any monitoring for their own good.
Answer: B

但 Apple 也明确写:Apple Watch cannot detect all falls.

sensor → algorithm → device → network → contact → response
Which of the following is the best title?
A. Smartwatches Can Now Prevent Every Fall
B. Can a Watch Catch a Fall? Why Safety Technology Must Work Outside the Laboratory
C. Older Adults Cannot Learn to Use Wearable Technology
D. Scientists Have Built a Perfect Fall Detector
Answer: B
A:detection ≠ prevention;C:ageism;D:不存在 perfect detection。
1. detect a fall ≠ prevent a fall
2. fall prediction ≠ fall detection
3. high accuracy ≠ high sensitivity
4. laboratory performance ≠ real-world performance
5. simulated fall ≠ real older-adult fall
6. false positive ≠ false negative
7. technology available ≠ technology used
8. user does not use it ≠ user cannot understand technology
9. safety monitoring ≠ unlimited surveillance
10. smart device ≠ complete care system(最后一组尤其重要)
1. fall — suffer a fall,
2. detect — detect a hard fall,检测。名词:detection。
3. predict — predict future fall risk,预测。名词:prediction。
4. sensitivity — high sensitivity,
5. specificity — high specificity,
6. false positive — generate a false positive,
7. wearable — wearable technology,
8. adherence — long-term adherence,
A fall detector has 99% accuracy, so it must be excellent. TRUE? FALSE?
DETECTION(Has it happened?)、PREDICTION(Will it happen?)、PREVENTION(Can we stop it happening?),让学生举例。
An ________ wearable has zero sensitivity. Medical technology is not only an engineering problem. It is a human-behavior problem. 结束。

CDC|2026 — Older Adult Falls Data
(cdc.gov/falls/data-research/index.html)

(pmc.ncbi.nlm.nih.gov/articles/PMC12609574/)

Frontiers in Public Health|2026 — Accuracy of wearable devices in predicting falls in older adults: a systematic review and meta-analysis
(pmc.ncbi.nlm.nih.gov/articles/PMC13015825/)

Journal of Geriatric Physical Therapy|2014 — Fall Detection Devices and Their Use With Older Adults: A Systematic Review(Shomir Chaudhuri, Hilaire Thompson, George Demiris)
(pmc.ncbi.nlm.nih.gov/articles/PMC4087103/)

Measurement|2026 — Pre-impact fall detection with wearable sensors: Datasets, models, and sim-to-real generalization — A systematic review

BMC Geriatrics|2024 — Exploring factors affecting the acceptance of fall detection technology among older adults and their families
(link.springer.com/article/10.1186/s12877-024-05262-0)

Older Adults' Experiences With Using Wearable Devices|2021
(pmc.ncbi.nlm.nih.gov/articles/PMC8212622/)

Apple Support|2026
(support.apple.com/en-sg/108896)

而是:Detection ≠ Prevention;Lab Accuracy ≠ Real-World Reliability;Available Technology ≠ Adopted Technology;Safety ≠ Unlimited Surveillance。

What does the older person actually want?
